I appoint the Board of Directors of BluPeak Foundation to appoint a proxy to represent me at all meetings of the members of this Foundation. This proxy will vote on all matters, give consent, and act in my place and stead. This proxy remains valid for three (3) years from today unless revoked by me in writing or by subsequent proxy. This proxy will be void if I attend and vote at any meeting.
